Calmora Health Group is developing a network of six-bed Congregate Living Health Facilities in Southern California — 24-hour skilled nursing, ventilator support and complex care planned in home-like residences.

01 Our mission
A Congregate Living Health Facility (CLHF) is a licensed residential healthcare facility for patients whose needs are too complex for ordinary residential care, but who generally no longer require an acute-care hospital. Care is more medically intensive than a typical skilled nursing setting — 24-hour skilled nursing, ventilator support and management, tracheostomy care, complex wounds, IV therapy — delivered in a six-bed residence where every patient is known by name, by history, and by what matters to them.
A CLHF is not assisted living, a board-and-care home, a nursing home, a hospice residence, or a private home with caregivers. It is a regulated healthcare facility where medically complex residents receive continuing nursing and clinical support in a residential setting.
Our mission: to provide compassionate, clinically responsible, and individualized care for medically complex individuals in a safe, dignified, and home-like residential environment — combining the clinical discipline of a licensed healthcare facility with the personal attention a six-bed residence makes possible.

04 Clinical programs
Ventilator support and management, tracheostomy care and suctioning, airway management, oxygen therapy, and respiratory monitoring — in coordination with respiratory therapists, pulmonologists, and equipment providers.
24-hour skilled nursing: assessments and monitoring, medication administration and management, complex wound care, catheter and ostomy care — with IV therapy and line care based on physician orders and facility capability.
Physical, occupational, and speech therapy — provided or coordinated according to each patient's individualized plan of care. Progress is measured in what matters: a first step, a first word, a meal at the table.
Enteral nutrition and dietary support, social services, recreational programming, family education and care conferences — and discharge and transition planning from the day of admission.
Proposed patient profile. Every patient undergoes an individual clinical review before admission — the ability to admit depends on licensure, physician orders, staffing, equipment, and bed availability.

CALMORA's leadership brings experience across healthcare operations, healthcare-facility development, regulatory and licensing coordination, post-acute and long-term care, hospice and palliative care, and complex wound-care operations. The standard is simple: every protocol clinically responsible, every room filled with daylight, every family greeted by someone who knows their name.

06 The road to opening
Projected timelines are management estimates and are subject to changes in construction, permitting, inspections, utility work, licensing, staffing, equipment procurement, payer enrollment, financing, and other regulatory or operational conditions.
07 Admissions
Every referral follows the same structured path — so hospitals, families, and health plans always know where things stand.
A hospital, case manager, physician, health plan, family member, or authorized representative contacts Calmora.
Records are submitted securely. A qualified clinical professional reviews medical condition, nursing and respiratory needs, medications, nutrition, equipment, rehabilitation needs, and safety risks.
We confirm whether the residence has the license, bed availability, staffing, competencies, and equipment to care for the patient safely.
Eligibility, authorization, benefits, and payer requirements are reviewed.
On acceptance, we coordinate physician orders, pharmacy, respiratory and durable medical equipment, nutrition, transport, and the discharge plan.
The nursing and interdisciplinary team completes required assessments and begins the individualized plan of care.
All payer participation is subject to enrollment, contracting, authorization, and applicable benefits.
08 For hospitals & case managers
We welcome pre-opening referral inquiries. We aim to acknowledge inquiries within one business day; the time required for a complete clinical review depends on the availability and completeness of medical records, patient complexity, bed availability, staffing, equipment requirements, and payer authorization.

Demolition is complete and framing is open at the Skyland Drive residence, with bathroom reconfiguration in progress — the third and final home of CALMORA's first phase.
The Avenida Terrazo residence is fully renovated. Permitting and CDPH licensing preparation continue.
The second residence in the network — on Darwin Drive — is finished: interiors, kitchen and bathrooms complete. The home now awaits licensure.
With the Skyland Drive property acquired, CALMORA's initial three-residence development pipeline across Corona and Moreno Valley is complete.
The Avenida Terrazo residence becomes the first property in the CALMORA network — planned as a purpose-developed, six-bed Congregate Living Health Facility.
